Potential Fathers on Intake Forms and Client Records

Overview

Centers that track information about a potential father (common for pregnancy centers) can now capture details on more than one candidate, both when a client fills out an intake form and when staff work in the client record.

This section only appears when a form or client record includes potential-father fields. If your intake forms don't ask about a potential father, you won't see this.

What a client sees on the intake form

When an intake form includes potential-father questions, the Client Portal shows a Potential fathers section below the rest of the form:

  • The client can fill out one card of information per potential father: name, age, relationship, how long they've been in the relationship, whether he knows about the pregnancy, whether he is involved in the decision, and plans for the future together.

  • Add another potential father adds a new, blank card so the client can enter more than one candidate.

  • Each card is numbered (Potential father 1, Potential father 2, and so on). The first card is labeled Primary.

  • Remove deletes a card. It only appears once there is more than one card, so at least one card always remains.

Required fields work the same as the rest of the form: a candidate's required fields must be completed before the client can submit.

What staff see on the client record

Staff can view and manage the same information from a Potential Fathers panel on the client record:

  • Each potential father is listed with their name, a Primary badge on the primary candidate, and their relationship to the client if one is on file.

  • Quick facts (age, relationship length, whether he knows about the pregnancy, whether he's involved in the decision) are shown under each entry.

  • Staff can:

    • Add another potential father to create a new entry.

    • Click the edit icon to update an existing entry.

    • Set primary on any non-primary entry to change who is marked primary.

    • Reorder entries with the up/down arrows.

    • Delete an entry.

If no potential fathers have been recorded yet, the panel shows "No potential fathers recorded" with a prompt to add one.
