On a client's Visit workspace, open the counseling chart (for example Client Advocacy) and expand the Counselor's Assessment section, labeled "Counselor's assessment of risk of abortion."
This section records the counselor's professional assessment of abortion risk based on the client's situation during the visit. It updates the Initial Assessment field on the linked case. It is separate from the client's own stated intentions after the visit, which are captured in Pregnancy / Intentions.
Checkboxes are grouped into three categories, each shown with its own explanation and with an on-screen description under every checkbox:
The client appears to be planning or intending to obtain an abortion. Check any that apply:
Seeking an abortion: She is seeking information as to how to obtain an abortion.
Abortion is scheduled: She has an abortion scheduled, regardless of how tentative she seems.
Abortion procedures have begun: The abortion procedure has been initiated (for example, laminaria or the first pill in a medication abortion protocol).
The client faces obstacles that may make continuing the pregnancy feel difficult or impossible. Check any that apply:
Has not eliminated abortion: She has not eliminated the possibility of abortion.
Is pressured to abort: She is being pressured to have or consider an abortion.
Is undecided: She is undecided. This may be expressed verbally or on the intake form.
Has a medical condition: She is against abortion, but she has a medical condition she thinks may affect the pregnancy.
Does not meet criteria for likely to carry: She does not meet all criteria for likely to carry, or any criteria for abortion minded.
The client does not meet abortion-minded or abortion-vulnerable criteria. Check all that apply:
She has stated that she has made a decision to carry to term.
All indications reveal a healthy pregnancy.
Has significant support: She has support from significant influences in her life (partner, parents, close friends, or others).
Above the categories, the Initial Assessment field shows the category (for example "Abortion Minded") derived from the checked boxes. This value is synced to the linked case and is read-only in the workspace.
These assessment checkboxes are tied to the case, not the visit, but they now lock in step with the counseling chart's review status:
When the counseling chart is sent for review, CoolFocus places a matching lock on these case fields (Initial Assessment and the underlying checkboxes) in addition to locking the visit section. Trying to save a change to them while the chart is in review is blocked, the same as any other locked field.
If the chart is returned for corrections, this case-level lock is released automatically along with the visit section, so the fields are editable again while staff make corrections.
Once the chart is approved and signed, these case fields are re-locked to match the chart's final locked state.

These checkbox labels and descriptions carry forward the original CF3 counselor assessment wording, so long-time staff see familiar language when the client's situation matches one of these categories.
