When a client taps the client form or consent link from a text or email, CoolFocus asks for the client's last name and date of birth first, before showing any questions.
The link only opens once the last name and date of birth match the client record it was sent for.
This protects the form if the text is forwarded or seen by someone else, since a name and phone number alone are not enough to open it.
If a client says the link won't open, confirm their last name and date of birth are correct on the client record, then resend the form.
When a client has already completed an client form or consent, staff can open the submitted responses from the appointment (before starting the visit). See Viewing submitted client forms from an appointment.
